Abstract

Inflammatory bowel diseases (IBD) are chronic recurrent immune-mediated inflammation of the gastrointestinal tract. At this moment there is no unified system of monitoring patients with IBD in Russia, but some information from different regions about the prevalence of disease is available. Also, there are the registers in few big cities. The IBD registers have been conducting in Novosibirsk since 2003. The data from this register can give us the possibility to estimate the various real-time data of the disease, to analyze the effect of the treatment, to evaluate risk factors for the disease in real clinical practice. According to the register data the number of patients with recurrent and continuous forms among the patients with ulcerative colitis is decreased, the number of patients with distal forms is increased, which is probably due to the more skillful patients conduct. In general, a number of patients with Crohn’s disease with continuously relapsing course decreased, reflecting, perhaps, improved treatment. Also the number of patients with affected small intestine increased. This fact is associated with greater availability of diagnostic methods of the small intestine. Conducted molecular-genetic testing aimed at the identification of allelic variants of polymorphic markers of genes: tumor necrosis factor a -TNF-a (-G308A), monocyte receptor CD14 (-C260T), cytotoxic T-lymphocyte-associated protein 4 - CTLA 4 (-A49G) that lead to an increased risk of CD and UC, allowed to create database for the development of methods of forecasting occurrence, disease course and IBD therapy optimization.
