The analysis of truncus arteriosus surgery experience

Abstract

The study purpose is analysis of 15 years experience of truncus arteriosus surgical correction. Materials and methods. Total 45 patients at the age of 5 months to 18 years (10,28 ± 6,15 months), 21 – male patients, 24 female were operated. Results. The radical correction of trunkal and atrio-ventricular valves without intervention was carried out at 35 patients (76.1 %), surgeries with the plastic reconstruction of rear wing trunk non-coronary valve – in 4 cases (8.7 %), and operations with plastic reconstruction of trunkal and atrio-ventricular valves – in 7 cases (15.2 %). Homografts were used in 12 patients (26.1 %), xenografts – in 34 patients (73.9 %). Mortality within the hospital period was 28.3 % (13 cases), it was 31.4 % in 1998–2009, and it decreased to 18.2 % in 2010–2012. The cardiovascular and respiratory failure were revealed in 28 cases (60.9 % of patients), cardiac arrhythmias – in 10 patients (21.7 %), multiple organ failure – in 6 patients (13.0 %), residual pulmonary hypertension in 4 patients (8.7 %). 29 patients were alive after 5 years period among 32 patients with a favorable outcome in the hospital, the actuarial survival rate was 90.6 %. The reoperation was performed at 13 patients (54.2 %) from the 24 patients, among them the one reoperation was performed in 10 cases (41.7 %), the 2 reoperations were carried out in two cases (8.3 %), one patient (4.2 %) over went 4 reoperations.
