TREATMENT OF BONE MARROW EDEMA FEMORAL HEAD BY INTRA-ARTICULAR INJECTIONS PERFTORAN WITH DIMEXIDUM. CLINICAL CASES

Abstract

Aim of the study – to assess the effect of intra-articular introduction of perftorane with dimexide on the hip bone marrow edema diagnosed by MRI as a stage preceding the development of necrotic changes in the head of the hip joint. Material and methods. The paper presents the results of conservative treatment of 9 patients with bone marrow edema of the femoral head (0–I stage of aseptic necrosis, MRI), which underwent the courses of intra-articular injections of perfluorane-dimexide mixture under ultrasound guidance. Assessment of the clinical and functional state of the joints was carried out according to the Harris scale. Results and discussion. The only positive dynamics was observed throughout the year in all nine patients included in the study as the result of the treatment of bone marrow edema by intraarticular injections of perftorane-dimexide mixture, which was reflected in MR-tomograms by reducing the zone of bone edema. The complete removal of edema (77.8 %) was achieved in 7 out of 9 patients after two courses of injections of perftorane with dimexide; in two patients (22.2 %) – after three courses. Conclusion. The course of intraarticular injections of the mixture of perfluorane-dimexide prevents the development of structural necrotic changes in the hip joint, allows preserving the integrity of the joint and significantly postponing surgical intervention on the pre-roentgenologic stage of aseptic necrosis of the femoral head, the stage of the bone marrow edema.
